Henderson, Human Services Administration, Bureau of Motorist Compliance, Division of Motorist Services, dated 02/09/2018 concerning my drivers license renewal for 2017. I took a HSMV Form # 72010 signed by my eye doctor to the Milton, Florida DMV office and was issued a drivers license on 11-06-2017 with the expiration date of 07-22-2023.
