You can make an in-person Louisiana Driving Records Request by visiting the Office of Motor Vehicles (OMV), filling out and submitting the ODR request form and paying $ 16 driving record fee.

By mail

You can request your Louisiana driving record by mail by:

You can make an online Louisiana Driving Records Request by:

• Providing your name, address, DOB, driver’s license number, license class and mailing address
• Paying through debit or credit card a $ 16 online driving record fee plus an additional $ 2 online processing fee
