You can request your driving record by mail. You will need to complete Form MV DPPA1, which is a Request For Motor Vehicle Record. This will require you to fill in your name, license number, date of birth, and social security number. You must include a certified check or money order for the driving record fee. To request a driving record in person you will need to go to a local Alabama Department of Revenue (ADOR) office. You will be required to fill in the Form MV DPPA1 and provide payment to the representative during the time of processing your request.
