Oklahoma became a state in 1907 and it began maintaining vital records such as birth and death records in 1908.
In 1917, the state of Oklahoma made the filing of birth and death records mandatory. However, it wasn’t until the 1940?s that the filing of birth and death records became a norm. Today, the Vital Records office of the Oklahoma State Department of Health maintains the records of each and every death that takes place in the state. Provided you’re an immediate family member of the deceased or someone with a tangible interest in the record, you can easily obtain a person’s death record by contacting the Oklahoma Vital Records Service. Two types of death records can be obtained from the Vital Records Service: open records and sealed records. Following is what each type of record refers to.
Are Oklahoma Death Records Public Record?
This is the Oklahoma death record made available to the eligible individuals. The Oklahoma Vital Records Service maintains open death records of the previous 75 years. To obtain this type of death record, you need to provide proof of eligibility and photo ID, complete the application, and pay the fee.
Oklahoma Sealed Death Records
These records are known as sealed records because they require a court order to open. In short, you can obtain this type of death record only if you have the relevant court order.

My grandmother died in Muskogee, Ok in Sept. 1936. I have looked every site I can find for her maiden name. Would her death certificate have her maiden name on it or only her 3 married names? Also would a copy oh her death certificate still be avaliable?

How do I get the application to request a copy of a death certificate in Oklahoma?
I need one for my great aunt Malvina (Reed) Large, born Oct. 8, 1879 and died in June 1962 in either Venita, OK or in Okmulgee, OK. I am 77 and do genealogy for my family. Malvina was the sister to my grandmother and is the only one I have not been able to get her exact death date and where she is buried there in OK.
