Iowa Driving Records By Mail

You will need to start by completing a Privacy Act Agreement For Request of Motor Vehicle Records (Form 431069). If you are requesting just your own driving record, you only need to fill in Sections A and D. You will need to take this agreement and your driver’s license to a local Iowa DOT office.

If you decided to request your Iowa driver record by mail, you will need to include the agreement form, a copy of your driver’s license, and payment for the service. Both in person and by mail fees are $5.50. You can mail your request to the Office of Driver Services at the Iowa Department of Transportation.

Iowa Driving Records Online

You can easily order a certified copy of your driving record online. Go to the Iowa DOT website. You will need to supply your name, birth date, social security number, and driver’s license number. The fee is $7 and can be paid with a debit or credit card.

If you are just ordering a copy for your own personal use, we suggest getting the non-certified copy. This is available online at no charge for all residents of Iowa. Keep in mind that non-certified copies are not official documents that can be used for legal purposes.

    At the present time I live in Ontario, Canada. I was initially licensed to drive in Iowa in 1968. For reasons passing logical understanding the Province of Ontario needs me to prove that I was licensed at that time. As you can expect I don’t have a lot of documentation from 52 years ago. I know I completed my driving tests at the Ida Grove Licensing office in Ida County. Would you have any record that documents my licensing at that time? If you have such a record how would I access it?
