  1. First, you must have your current official birth certificate, or order a new one before entering this process. 
  2. You must also have a valid reason to make a change to your birth record information such as an adoption, a name change, court order, or a clerical error. 
  3. Gather all the supporting documents for the name change and have them ready to submit for your petition.

    The state of IL will not give you a copy of a birth certificate if your first name is not on the original document. For example from my experience with Cook County IL.I was trying to get my uncle Orvall Sylvester Cambron’s birth certificate, but they would not send me a copy because my first name (Gloria)was not on the original. My yuncle was born in 1916 and of course I was born 1934. If you know thatYOU HAVE TO HAVE YOUR NAME ON THE ORIGINAL, I HOPE THAT YOU MAY HAVE TO KNOW WHO WAS LISTED AND IF YOU USE THE NAME.THE MOTHER’S or father’s NAME, you might be able to buy the document for $15 Let me say if they question you about your answer, there is no guarantee you will be successful.     I need to know how to search for a record of birth with only having the mothers name and a date range and the state and possibly the county. It is for a client’s last Will and Testament The mother can’t be located after a prolonged search. We do know // presume she married sometime after the child’s birth. The child may also have been put up for adoption. We are trying to track down the birth certificate. An additional complication is that the mother of the child may have been adopted as well. We know nothing about the mother’s adoption nor the family that adopted her other than their last name and approximate location as we know the school the mother attended.
