Available Arizona Driving Records

Driving records in the state of Arizona are available at local DMVs, by mail, and online. Depending on the type of record you want, you may be limited in which places you will be able to obtain your record from. Let’s take a look at the routes you can take for getting your driving record.

Arizona Driving Records By Mail

You can obtain a driving record through the mail. The types of records you can request are the 39 month certified, the 5 year uncertified, and the 5 year certified. You will need to complete the Motor Vehicle Record Request (Form 46-4416). This form will need to be notarized.

You will need to send the completed form along with payment to the Arizona Motor Vehicle Division. You must pay with a check or money order when submitting your application by mail.

Arizona Driving Records In Person

You can request your driver record in person at any one of the local Arizona DMV facilities. You will need to fill in the Motor Vehicle Record Request (Form 46-4416). An agent at the facility will need to witness you signing the document and will ask for photo identification. You can pay with cash, check, money order, and at some locations, you can pay with a credit card.

Arizona Driving Records Online

You can obtain only the 39 month uncertified driving record online at the official Arizona Government Website. You will be required to enter and confirm your personal information when ordering online. After submitting payment via a credit or debit card you can immediately print out your driving record.

    Arizona Driver Abstract copies can be obtained from the Arizona Department of Transportation – Motor Vehicle Department. You will need to fill out the ADOT Motor Vehicle Record Request Form and have your signature notarized or witnessed by an MVD agent.

    How To Get An Arizona Driver Abstract

    1. Visit the Arizona MVD website (azdot.gov)
    2. Complete the Motor Vehicle Record Request Form
    3. Have your signature notarized or witnessed by MVD agent
    4. Pay the $5 fee for a certified copy
