How to Alaska Obtain Death Records

To obtain death records, you must first fill out this form:

It’s helpful to provide the name of the decedent’s mother and father and the decedent’s date of birth to make it easier find an accurate record. To obtain a certified Alaska death certificate you must be the decedent’s spouse, child, parent or sibling. You must provide proof of the relationship along with your application. If you’re a sibling, you’ll need to provide a copy of your birth certificate showing that you and the decedent have the one or two of the same parents. Certified Alaska death records can also be provided to a representative if they have proof showing that they need the death certificate for a specific reason. The legal representative will need to provide a letter stating the person you represent and how you are related to the person on the death certificate. Before you send your application, be sure to provide photo identification in the envelope. Your photo identification needs to readable and clear. If the photo identification doesn’t meet this requirement, your application won’t be processed. A driver’s license or a government issued identification card is acceptable for identification purposes if it’s unexpired or expired for no longer than one year. The identification can be from any state in the United States.An unexpired United States passport, foreign passport, United States military identification, military dependent identification or veteran’s identification is also acceptable for identification purposes.

If you’re living in Alaska, tribal identification cards are acceptable for identification purposes as well.

If you can’t provide identification, you’ll need to contact the Alaska Bureau of Vital Statistics to speak with a customer service representative at 907-465-3391.

You can mail or fax your application to their office. You can also visit their Anchorage or Juneau offices to submit your application.

How Much Do Alaska Death Records Cost?

Alaska charges $30 for the first copy of the death certificate and $25 for each additional copy of the same record requested at the same time. You can pay the fees with a credit card, or you can send a check or money order. The checks must have your name printed on them. You’ll be charged a $30 fee for returned checks. If a record isn’t found, you won’t be refunded the money you paid for the search.

Alaska has offered the option of choosing regular, priority, express or FedEx mail when you order the record. Regular shipping is free. If you choose the FedEx mail option, it can’t be shipped to a PO box. Other shipping fees are:

Express Mail: $22.95
FedEx Mail:$24.25

Alaska Death Records Processing Time

If you send your application through regular mail, it’ll take approximately two to three weeks to process your request. If you’d like expedited service, it’ll take approximately three days to process your request. If you’d like expedited service, you’ll need to include an additional $11.00 with your application. If you choose expedited service, you’ll need to fax the application to their office.

I am trying to verify the death of my estranged husband. I have not found an obituary or newspaper listing. But, found in 2 different people search sites that he died. His information is as follows:

George Albert Seward born November 11, 1952 and supposedly died December 8, 2015 in Fairbanks, Alaska.

Lila, we don’t show an Alaska death record for the person you mentioned in that time frame. Sometimes older Alaska death records are unavailable with our data resources. Our Alaska death records only go back around 40 years. That being said, you can contact the Alaska Department of Heath and Social Services. This is the Alaska state agency to contact for vital records which include death records, birth records and marriage records. You can call them @ 907-269-7800

i am searching for my brother Robert Willi Tettenburn. I haven‘t heard from him since 2004. He was born 01/30/1962 in Schweinfurt/Germany. He is american Citizen since birth. His last address I know of was 247 S. Franklin, Juneau . I need to know, if he is still alive or maybe he died, cause I haven‘t heard anything from him for so long. Hope you can give me information.
